  2. Print the completed form and SEND PAYMENT WITH YOUR NON-REFUNDABLE $25 APPLICATION FEE  HERE online or to the address at the bottom of the form. ‎

    My work may be used for publicity purposes.
    I would like to donate a piece of my artwork, the sale of which would further promote the Mt. Lebanon Partnership Arts Initiative.
    I am interested in a beginning or end of row booth, if available, for an additional $25.

    Three images of representative art work (Jpeg Form ONLY) are required, as well as an image of your fully stocked tent. NO ONE WILL BE ACCEPTED WITHOUT A TENT SHOT. Applications will be considered incomplete without images. Please email these to
